For First Time Patients:
You will need to fill out some paper work. (Please plan for 1 ½ hours for your first visit.)
Three forms are available here. If you are able to download and fill them out before your first treatment, that would help to stay on schedule. If you cannot download, you can come to the clinic 15-20 minutes early and fill them out there.
HEALTH HISTORY CONSENT FORM FINANCIAL AGREEMENT
- Wear loose, comfortable clothing. Since most acupuncture points used are on hands, arms, legs and feet, please wear loose clothing that can be pulled up above the elbows and knees. If you cannot wear loose fitting clothes to your appointment, you can bring a change of clothes with you. Please don’t wear any perfume or strong scents the day of your appointment, as some patients have allergies. Snack or eat at least 1-2 hours before your treatment. Acupuncture is not advised on an empty stomach.
- If this is your first appointment, have a seat until the acupuncturist comes to get you. You will have an initial private consultation in which you will discuss your health history and current health concerns. The sliding scale fee structure is explained and payment is made at this time. If you like, you can also schedule future appointments at this time.
- All treatments are paid for by either checks or cash only, please. The clinic has a self-payment and scheduling station called an Invisible Receptionist (IR). For all appointments after your first one, this is where you pay and reschedule yourself.
Invisible Receptionist (IR):1. When you arrive at the clinic, go to the IR station in the reception area. There you will find an envelope with your name on it. Check inside the envelope for your progress note.
2. Fill out your progress note and bring it with you to your chair.
3. Place payment (checks or cash only please) into the envelope and drop the envelope into the payment box.
4. You will also find a scheduling book. Sign yourself up for your next treatment at this time. Getting the paperwork and scheduling out of the way beforehand makes it easier to relax into your treatment and take that relaxation with you as you continue your day.
5. Please turn off all electronic devices, such as cell phones, before entering the community treatment room. It is also important to keep conversations minimal and whispered.
6. You can then go into the community treatment room to select a chair and blanket and get comfortable. Remove your shoes and socks. Take off any jewelry that may get in the way of treatment and put it into the basket next to the chair. Roll up your sleeves and pant legs. Relax into the quiet and I will be with you shortly.
Length of the treatment: Treatments last anywhere from 30 minutes to 2 hours. You can stay as long as you want. Some patients fall asleep. If you need to leave by a certain time, please let me know. If you need anything at any time during your treatment, open your eyes and give me a meaningful glance; I will be in and around the treatment area keeping an eye on things. It’s possible someone may begin to snore during the session. If this would bother you, earplugs may be a good solution.
Course of Treatments:Acupuncture works best within a series of treatments closer together rather than weeks or months apart. This is what makes the sliding scale fee so practical. On your first visit, I will recommend a course of treatments best suited for your complaints. This may be 2-3 treatments per week for 1-2 weeks or perhaps 1-2 treatments per week for a longer period of time. One treatment per week is usually the minimum amount of visits to experience lasting results. Many people decide after the initial course of treatments to continue treatments on a periodic maintenance schedule to maintain good health.
Fee Policies:The cost of treatments is $15-40. You determine the amount based upon what you can afford considering the number of treatments needed. Because of the sliding scale, I cannot bill insurance companies. If you have a health savings plan, an insurance policy that covers acupuncture treatments, or a Flexible Spending Plan, I will be happy to give you a receipt that you can submit.
For any appointments that are rescheduled or canceled within 24 hours of your appointment and for appointments missed without notice, a $15 fee will be charged. Fees will be waived, of course, for severe weather conditions. It is important to arrive on time for all appointments.
